COVID-19 Acknowledgement Forms
For the 2020-2021 school year, Minuteman High School is requiring all students, parents/guardians, and staff to sign a form acknowledging that students and staff will not come into the Minuteman building if exhibiting the listed symptoms related to COVID-19, and will follow the listed health and safety protocols.
Minuteman emailed the forms to students, parents, and staff the week of September 18, to electronically sign.
